1. Apply for Form DS-11: You can apply for this form if
• You are applying for U.S. passport for the first time.
• Your age is under 16.
• Your earlier passport was applied when you were under 16.
• Your passport is damaged or got stolen.
• It has been more than 15 years since your first passport was issued.
• If you don’t fall in any of the above-given conditions then you can renew your passport by filling the form DS-82 online.
• Fill the form, but do not put your signature until the time your acceptance agent doesn’t instruct to do so.
• With the form You need to present a Social Security number, This number is issued, in accordance with 22 U.S.C. 2714a and 22 C.F.R 51.60(f).
• If the social security number is not being issued then you need to submit a statement, duly signed and dated, including this phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security Number by the Social Security Administration.”
• You can also apply for a larger passport book without spending any extra penny.

2. Evidence of US Citizenship: After filling the form you need to provide the evidence of US citizenship. Some of the original documents which can be presented for that are
• Birth certificate issued by a US hospital which should include your full name, date of birth, and place of birth, parent(s)’ full names. It should be issued by city, county, or state of birth, and should have the registrar’s signature, and the seal of the issuing authority.
• Consular Report of Birth Abroad or Certification of Birth
• Naturalization Certificate
• Citizenship Certificate
• Bring the photocopy or a second certified copy of these documents.

3. Get your ID proof: You should present your original ID proof as well as the photocopy. It may include the following
• Valid, U.S. passport
• U.S. driver’s license (fully valid)
• Certificate of Naturalization or Citizenship
• Government employee ID (city, county, state or federal)
• U.S. military ID or military dependent ID
• Valid foreign passport
• Mexican Consular Identification

4. Provide your recent photo: All you need is 2 passport size photographs which are a 2” by 2” coloured photograph.

5. Passport Fee: Calculate your passport fee which is different for different individuals.
